To add/change the header/footer: open the plugin drop-down menu and select the option for changing the header/footer.
To enter a string for the header/footer: select "Use header/footer string" for the "Header/footer type," and enter the header/footer in the "Header/footer string" field.
To import the header/footer from a file: select "Use header/footer file" for the "Header/footer type," and select a file that contain the header/footer text in the "Header/footer file" field.
To remove the header/footer: select "No header/footer" for the "Header/footer type" drop-down menu.
To copy the text in the Output window to the Editor window: invoke the plugin by clicking its button or selecting its primary menu option.
